HDS Individual and Family Dental Plan

Whether you're retired, self-employed, working full or part-time without dental benefits, or no longer covered through a parent's plan, HDS's individual and family dental plans can pay big dividends for oral health while making a lot of sense financially. Take advantage of one of our individual plans designed to help protect your smile---and your pocketbook.

HDS offers two individual plans, the Individual Plan and the Individual Plan Plus for Hawaii residents. Both plans provide 100 percent coverage for preventive services such as oral exams and cleanings in addition to fluoride and sealants for children. The plan maximum for both plans is $1,000. HDS will work with you to tailor your plan benefits to fit your needs. To obtain a proposal, complete the Request for Proposal form and attach a summary of your current dental benefits, utilization and experience reports. Send the completed form and attachments to HDS, 700 Bishop Street, Suite 700, Honolulu, HI 96813. An HDS representative will contact you after receiving your information.
